After splitting from her boyfriend junior doctor Ruby Walker seeks a new start away from England and answers an advert to work at the busy, under-funded Good Karma hospital in India, run by straight-talking Doctor Lydia Fonseca, along with elderly surgeon Ram Nair and the arrogant younger Gabriel Varna. Ruby wins her spurs dealing with a difficult pregnancy but is overwhelmed by the volume of patients, including Maggie Smart, who is in the country with husband Paul for their daughter's wedding and has a brain tumour. When the team fails to save two brothers brought in suffering from stab wounds Ruby considers quitting but is dissuaded by Lydia - who is not amused to find her occasional boyfriend Greg McConnell has donated an ambulance advertising his bar. A muscular young man, totally nude and suffering from amnesia, is dragged from the sea and taken to the Good Karma hospital, where he gives no clues to his identity. Ultimately Lydia discovers his name and his secret and, helped by Ruby, makes him face up to his situation. Ruby also visits a village school to give the children their medicals and is asked by teacher Anton Karkik for a date, which she turns down, and to help pupil Vijay, who is withdrawn and under-sized for his age. This leads Ruby to a confrontation with his single mother and the discovery of an incident in Gabriel's past. Gabriel himself races against time to collect the serum to save a roguish snake-charmer bitten by one of his animals and almost fails, following a road accident, whilst Maggie decides that she wants to die in India. A blast from the past for Dr. Fonesca reunites a father and son, but her diagnosis leads to more separation. An infant is put at risk through good intentions, but results in a situation where people in authority are unwilling to contemplate other options. AJ is caught up in his pretend doctor's life and finally sees the harm that he can do to innocent people who believe in him. Ruby clashes with Gabriel who accuses her of being naïve in not recognizing that British tourist Vicky Martin has come to India for a cheap and illegal organ transplant whilst Lydia ejects Bina Barros, a celebrated Bollywood singer, for demanding drugs without a prescription. However nurse Mari, a devoted fan of Bina, secretly smuggles the drugs out of the hospital to give her. Elsewhere Ruby attends street-sweeper Rajit Dave, knocked over by a hit and run and with no insurance, likely to lose his leg and his job. The car turns out to belong to eminent surgeon Varun Kapoor and Lydia makes sure he saves Rajit's leg. Lydia and Ruby visit a convent where they diagnose novice Sister Inez as being pregnant and, although she claims it is an immaculate conception, she is excluded from the order by severe Mother Carmen. When it turns out to be a phantom pregnancy Lydia tries to persuade her to move on but comes into conflict with Ruby after Inez takes drastic action. Ram brings in charming marriage broker Mala to supply a wife for an unwilling A.J. who is also unhappy to find Josie, the girl he lied to, working for Greg. As Ram finds romance with marriage broker Mala Pradeep Ruby unites with Marcus and decides to travel the country with him, quitting her job. Soon however she is involved with helping the victims of a coach crash, leading to a return to the hospital and praise from Lydia. Maggie's daughter Debbie is angry to learn that her mother wants to remain in India to die rather than leave for England with her but Gabriel steps in to restore family harmony and persuade Debbie to respect her mother's wishes.
